On Day 1 of QMIHSC, we are bringing the industry together for an honest and constructive panel discussion on the health and safety challenges shaping our future and how getting back to basics could be the key to moving forward. With a sobering list of major incidents and serious accidents in recent years, there is growing recognition that the fundamentals of strong planning, supervision, training, communication and leadership are more important than ever. Our expert panel features representatives from the Regulator, MEU, AWU, leading mining companies and major contractors, alongside Ken Singer Acting Commissioner for Resources Safety and Health QLD, to unpack lessons learned, explore evolving risks and reflect on what is needed to rebuild a safer, healthier and more resilient industry. It’s time to reconnect with the basics, challenge the status quo and commit to doing them well - the future of our industry depends on it!

Mr Ken Singer

Biography

Ken Singer is the current Acting Commissioner for Resources Safety and Health. Ken’s experience in the resources industry spans 4 decades across the coal mining, mineral mining and explosives sectors. Ken holds a Bachelor of Engineering in Mining Engineering, Graduate Diploma in Mine Ventilation and a Master of Science in Human Factors and System Safety. He also holds a first class mine managers certificate of competency for coal mines and a site senior executive practicing certificate. He has held senior operational roles, technical roles and executive roles throughout his career and previously held the roles of Chief Inspector and Deputy Chief Inspector of Coal Mines in Queensland from 2010 to 2013. As Commissioner, Ken aims to share his experience and knowledge for the benefit of others and one of his key priorities is to keep strengthening existing tripartite industry consultative arrangements between operators, workers and the regulator.

Biography

Hermann provides strategic leadership and direction through a focus on regulation and education. His inspectorate assists the industry to meet its obligations while promoting the safety and health of workers to achieve a goal of zero serious harm. He has over 40 years' experience in the extractive mining industries, in both surface and underground operations, and as a government regulator. His extensive experience is backed by qualifications that include a Diploma of Engineering, Diploma of Government Workplace Inspection, Fellow of the Institute of Quarrying Australia and a Certified Practicing Quarry Manager. His conviction that a zero serious harm industry is achievable keeps him focused.

Mr Jacques Le Roux

Biography

Jacques has a Master of Business Administration from MGSM in Sydney. Jacques also has a Bachelor of Mining Engineering. He is a Registered Professional Engineer of Queensland (RPEQ), a Professional Engineer of Australia (Pr. Eng) and is the current Chief Inspector of Coal Mines at Resources Safety and Health Queensland. Jacques joined Resources Safety and Health Queensland (RSHQ) as a Mines Inspector in May 2021. Prior to joining the regulator, Jacques worked as Operations Manager (Crinum Mine) and Mine Manager (Crinum, Angus Place, Springvale, and Clarence Colliery). Jacques also worked as a Mine Manager in South Africa and has more than 30 years’ leadership and management experience. Jacques has a Queensland First and Second Class Mine Managers Certificate and Opencut Examiners Certificate of competency. He worked as a Mining Engineer and undermanager at Central Colliery and as a Ventilation Officer at Grasstree . Jacques is passionate about people and achieving zero serious harm in our important industry.

Mr Gavin Lawrence

Biography

Gavin Lawrence brings over 20 years of experience across metalliferous mining and heavy vehicle mechanics, complemented by a strong foundation in trade qualifications and advanced training in risk management, safety and health investigations, and information communication. With extensive expertise as an underground maintenance technician and operational trainer/assessor, Gavin has been a dedicated advocate for worker safety and rights as an AWU Northern District Organiser in Queensland since 2016. In this role, he regularly represents employees in workplace investigations, safety and health matters, and incident resolution, while also assisting workers with workplace injury claims. Gavin actively contributes to industry governance and policy development through his involvement on multiple boards and committees, including the Queensland Mining Safety and Health Advisory Committee for Metalliferous Mining. In 2025, he serves as Chair of the Queensland Mining Industry Health and Safety Conference, continuing his strong commitment to driving positive change, improving safety standards, and advancing initiatives that ensure greater protection for Queensland mine workers.

Biography

Jason fisted began his career in the mining industry around 1986. His strong interest in health and safety led to his election as Miner’s Officer (later SSHR) in 1998. He went on to obtain a Third-Class Certificate of Competency (Deputy’s Certificate) and, driven by his passion for creating a safer industry, continued to progress his career. In 2012, he was elected to his current position as Industry Safety and Health Representative. He maintains strong beliefs in the importance of health and safety in mining.

Biography

Fiona is the Vice President of Sustainability at Evolution Mining. This is an Executive role with responsibility for the Health, Safety, Environment, Risk, Social Responsibility and Cultural Heritage portfolios. This includes leading the strategy and approach to Net Zero and ESG reporting. She is helping lead Evolution’s step change in Sustainability focussed on leaving a positive legacy within the communities in which we operate. After working in high-risk environments for more than 25 years, her ambition is to help build cultures where people go home better than when they arrived – where people and the communities are positively impacted. She is passionate about Sustainability, underpinned by a genuine care for people and the communities in which they live. Fiona is known for creating trusted relationships and connects strategy with delivery to help unlock potential. This care is anchored in her personal experience of a workplace fatality in her family - so her role is also a deeply personal one. Fiona’s operational experience is supported by a comprehensive academic background which includes a Masters in Risk and Technology, Post grad qualifications in Leadership, Transformation, Innovation and Coaching. She is also a published author in the areas of operational risk, contractor management and leadership.

With a career in mining spanning over three decades, Garvin has worked in the coal industry since 1995. Starting in electrical engineering, Garvin quickly expanded his expertise to include maintenance, production, and project management roles across both New South Wales and Queensland mines, with a particular focus on underground operations. His experience and leadership skills culminated in his appointment as Chief Inspector of Mines in NSW, a position he held from 2017 to 2023. During this time, Garvin played a pivotal role in ensuring safety and regulatory compliance within the industry. Known for his commitment to excellence and innovation, Garvin continues to be a respected figure in the mining sector, contributing valuable insights and guidance to ongoing projects and initiatives.

Steve Faulkner is the Head of Safety at Thiess, a major global provider of mining, industrial, freight rail, and transit solutions. Based in Australia, Steve leads the development and implementation of Thiess’s integrated Health & Safety Management System, critical safety controls, and strategies for both physical and psychological wellbeing across diverse operations and geographies. Holding qualifications from the University of South Australia, he draws on decades of experience in mining and heavy industry, shaping safety-first culture across the organisation.

Biography

Wayne has 30 years’ experience within the Underground Mining Industry. Throughout Wayne’s career he has held various senior management roles including Executive General Manager, Director, Mining Consultant and General Manager. Wayne’s area of expertise includes the implementation of leading strata management practices, implementation and management of material risk controls and reviewing mining performance & improvement opportunities. Wayne is a mad mountain bike rider, having completed in numerous 24 hour team races and other events. In recent years he has been riding in a 7 day event to raise money for the Westpac Helicopter, with the intention of doing many more.
